Nurse-Family Partnership Enrollment Application
Many first-time expectant moms have questions about pregnancy, their baby and parenting. Wouldn’t it be great to have a “personal” nurse for support and to answer those questions, free of charge? That's exactly what the Nurse-Family Partnership program does. This team of registered nurses helps first-time moms have healthy pregnancies, understand growth and development of their child, and helps moms set goals for themselves and their families.

Weekly-Biweekly Visits in Your Home

Beginning early in your pregnancy & continuing until your child turns two

A Registered Nurse will provide educational home visits on the following:

Prenatal Education

Self-Care and Infant Care

Building a Strong Support System

Child Safety and Child Development

Life Skills-including job training, education, and childcare

Help with obtaining Medicaid, CHIP, WIC, SNAP, etc.
